Haffenreffer's 20 Person-Centered Standards

The conscientious development and implementation of these 20 Person-Centered Standards will maintain a sustainable culture of quality care and services, as well as regulatory compliance. Our quality systems are based on the Baldrige Health Care Quality Criteria and the core values and concepts of person-centered care as found on the Pioneer Network web site.
Each “person-centered” standard focuses on the individual’s unique needs and characteristics. The standards are easy for staff members to understand, and serve as a guideline for developing and implementing policies and procedures, as well as a general standard for every daily task and interaction. Experience has shown that the standards are sustainable and adaptable to new and innovative practices. They lead to quality care, reduced staff turnover, higher satisfaction, fewer complaints and better surveys.
Each standard is supported with real world tools developed and tested by Haffenreffer & Associates including Assessments, Policies and Procedures, Forms and Inservice material. Many are available in stand alone versions, and we can customized any tool for a particular facility or situation.
Individual standards often overlap, and are best understood and most powerfully utilized when taken as a whole.

- Leadership Excellence. A system to ensure training in leadership that is provided at all staff levels. This includes how Senior Leaders deploy the organization’s vision and values; communicate with the; ensure legal and ethical behavior; provide for succession planning; and model behaviors that encourage high performance at all levels.
- Quality Improvement. This system includes strategic planning; strategy deployment; and process management. Measurement, analysis and improvement of organizational performance in the areas of care, customer service, workforce performance, leadership and finance.
- Corporate Compliance. This includes systems to ensure ethical and legal behavior as well as financial integrity, especially with skilled services and billing.
- Change of Condition. This includes systems for communication of resident changes in status and your staff’s ability to observe for, assess and appropriately follow up on these changes. Includes the skills to provide appropriate interventions and communicate needs to physicians and other practitioners, as well as families.
- Abuse/accident & incident investigation. This includes systems for reporting and investigating allegations of abuse and injuries of unknown origin in accordance with Federal and State requirements.
- Nutrition/dining. Person-centered systems for providing meals, 5-star dining programs, assessment and care planning for potential and actual nutrition problems.
- Skin prevention and treatment. This is a system for identification of risks, as well as systems for assessment, prevention and treatment of skin problems.
- Behavioral/psychopharmacological medication use. Includes a system for assessment of behavioral issues (including wandering and elopement) as well as the implementation of appropriate interventions that may include psychopharmacological medication use. Includes a system for appropriate use of psychopharmacological medications.
- Clinical Assessment and Care Planning. Systems for assessment (MDS and beyond) and care planning. This is to ensure all the resident needs are thoroughly assessed and interventions implemented as needed. Includes specialized nursing services such as IV therapy and tracheostomy care.
- Safety. This system includes infection control and immunization programs; environmental and preventative maintenance; fire safety; and disaster planning.
- Restorative/rehabilitative. Includes the provision of Rehabilitation programs, including Physical Therapy, Occupational Therapy and Speech Therapy; and Nursing Restorative Programs to provide quality services and to maximize services and reimbursement.
- Clinical record. This system includes paperwork flow as well as clinical record organization and storage; includes system technology.
- Activity. A system for the provision of an activity program that promotes self-esteem, pleasure, comfort, education, creativity, success and independence for all served.
- Social Service. A system for the provision of Social Services that improve physical, mental and psychosocial needs for all served; includes outside resources and discharge planning.
- Communication and training. This system includes communication with and among the workforce; communication with families and residents; communication with physicians; as well as the organization’s overall orientation and training program for staff at all levels.
- Physician. Physician services and standards for the Medical Director.
- Pharmacy services. Pharmacy services including medication delivery and storage.
- Diagnostic and outside services. This includes contracts and agreements with outside resources in the provision of diagnostic services or other care such as Dialysis, Hospice, Home Health or other care entities.
- Resident/Family/Employee satisfaction. The system for providing, obtaining, trending, analyzing and implementing action plans based on customer / stakeholder satisfaction surveys.
- Planning for a Sustainable Organization – The Future. This includes culture change initiatives and projects as well as community involvement such as volunteer programs or other community projects.
